*Additional Fees for rides outside a 25-mile radius will include a charge of $3.50 per mile. Radius is based on headquarters location at 6400 Coors Blvd NW, Albuquerque, NM 87120.

No Show/Cancellation Fees: Please provide notice of cancellation with a minimum of one-hour prior notice. A $50 fee will be charged for no shows or late cancellations.

General Provisions

Apollo Medical Transportation LLC will always try to use the most efficient route and will attempt to maintain schedules but will not be liable for delays caused by unforeseen circumstances beyond our control, such as weather conditions, icy/muddy roads or roadside accidents, construction, or closures.

Apollo Medical Transportation LLC will not be responsible for damaged, lost, or stolen items.

Apollo Medical Transportation LLC will only transport pets or animals that are certified therapy pets or service animals.

Apollo Medical Transportation LLC reserves the right to refuse services to a person or persons under the influence of intoxicating alcohol or substances; or who is incapable of taking care of self, or who shows conduct, which is such, or is likely to be such, as to make him or her objectionable to drivers or passengers. This does not apply to an ill person accompanied by an attendant or care giver.

Per NM State Regulation: 18.3.2.9(D)

Apollo Medical Transportation LLC may only transport passengers who do not require medical intervention to maintain their level of response, airway, breathing and circulatory status, with the exception of self-administered oxygen not to exceed six liters per minute via a nasal cannula; the oxygen container must be secured in accordance with other state and federal laws; and may not transport passengers that require medical monitoring or medical intervention.
